These consents need to be printed and signed with wet ink. Unfortunately these forms cannot be electronically signed. Once completed please scan and email to us at info@parrystreetfp.com.au
These consents will not be sent to your current medical practice until closer to the opening of Parry Street Family Practice, so that the records that are transferred are as up to date as possible.
